- use your NHS account (through the NHS website or NHS App)
- use your Patient Access account

Change of address
If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery. If you have moved outside of our catchment area the practice may recommend you register with a practice closer to your new address.
Changing your phone number or email address
Please contact the practice to change your telephone number or email address.
Changing your name
You will need to show us proof that you have changed your name. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
